GENERAL INFORMATION

Please provide FULL NAME as per AADHAAR CARD only.

Registration is open for all Categories of applicants, with the following criteria:

• "General Category" must provide the Indigenous Inhabitant Certificate Number issued by respective District Administration.
• "Pensioners" must provide their PPO Book Number
• "Government Employees" must provide their PIMS / Employee Code
• "Email" of any Family Member can be used to register on the CMHIS Portal.
• "Password" must be minumum 8 characters with a mixture of Alphabets numbers and special characters containing atleast One Uppercase, One Lower Case alphabet.
